US Navy orders 'operational pause' after 10 sailors missing in collision

WASHINGTON: The US Navy declared a worldwide pause in its operations on Monday (Aug 21) after the latest in a series of accidents left 10 sailors missing and five injured.

Chief of Naval Operations Admiral John Richardson ordered an "operational pause" to allow a "comprehensive review" of practices after the destroyer USS John McCain was hit by a merchant tanker off Singapore.

"As you know this is the second collision in three months and the last of a series of incidents in the Pacific theatre," Richardson said.

"This trend demands more forceful action. As such I have directed an operational pause be taken in all of our fleets around the world."

USS John S. McCain: Remains Found of Some U.S. Sailors Missing in Warship Crash
The remains of “some” American sailors have been found in sealed compartments aboard the USS John S. McCain, Adm. Scott Swift of the U.S. Pacific Command said Tuesday.

Swift said the Malaysian Navy, which has been involved in the search, has also located “potential” remains and they are working to confirm and identify those discovered.

The Navy vessel suffered significant damage to its hull when it was hit by the Alnic MC, a 30,000-ton chemical and oil tanker sailing under the Liberian flag.

Ten sailors have been missing since the incident which occurred Monday. Swift did not identify who or how many people the remains belonged to.

"Its premature to say how many and what the status recovery of those bodies is," he told reporters.

Divers find Remains of all Missing Sailors from USS McCain Collision

SINGAPORE – Divers have recovered the remains of all 10 sailors who went missing after the USS John S. McCain and an oil tanker collided near Singapore last week, the U.S. Navy said Monday.

Navy and Marine Corps divers had been searching in flooded compartments of the destroyer for days after the damaged ship docked in Singapore. The cause of the Aug. 21 collision is under investigation. The crash ripped a gash in the McCain’s hull, flooding crew berths and machinery and communications rooms.

The victims ranged in age from 20 to 39 years old and came from eight U.S. states..

USS John S McCain collision in Singapore waters due to 'sudden turn' by US warship, says MOT

SINGAPORE: A collision between US Navy destroyer USS John S McCain and an oil tanker in Singapore territorial waters on Aug 21 was due to a "sudden turn to the left" made by the warship.

This was the conclusion reached by the Ministry of Transport's (MOT) Transport Safety Investigation Bureau (TSIB) which released its findings on Thursday 8 March, following its safety investigation into the collision.

"The Transport Safety Investigation Bureau (TSIB) of the Ministry of Transport has concluded its safety investigation into the collision involving the Liberian-registered oil tanker Alnic MC (AM) and the US Naval vessel USS John S McCain (JSM) that occurred in Singapore territorial waters on Aug 21 2017," the statement said.

Ten US sailors died in the incident when the navy ship collided with merchant vessel Alnic MC (AM) east of Singapore at around 5.30am as it was heading to Singapore for a routine port call.

In its conclusions, TSIB said the warship "JSM made a sudden turn to the left into the path of AM because of a series of missteps that took place after propulsion controls were transferred."

The investigation revealed that:

a. JSM made a sudden turn to the left into the path of AM because of a series of missteps that took place after propulsion controls were transferred.

b. When the Bridge team of AM saw JSM turning, it presumed that JSM would be able to safely pass ahead. The collision happened within three minutes of JSM turning left, and the actions taken by AM were insufficient to avoid the collision.
