If they go for the an excision, Go Wide, Go Deep. It's easy to miss a small part of the problem.

The golden standard is to do the initial biopsy with an excision, however, if it's melanoma the golden standard is a re-excision where a specialized surgeon (not the dermatologist) will cut a second time around the initial cut as deep and as wide as possible.SoberAsABird wrote: ↑Sun Jun 05, 2022 3:58 pm If they go for the an excision, Go Wide, Go Deep. Quite frankly, I hope they win that bet.

You have to separate the initial biopsy / removal, usually followed up by pathology, from further treatment. The initial biopsy aims for full removal of the spot but only by the smallest margins. Since most bumps are not a melanoma they don't want to be that invasive initially. A typical biopsy in Cambodia will set you back 150$-200$ including pathology if a skilled doc does it. Before that, you pay for the initial consultation / check up, so add another 50 for that. Depending on the pathology there's a more expensive follow up or not. The follow up might be just a re-exision but that's the first step of the melanoma ladder. Higher up the ladder it soon starts to become very specialized and invasive with scans, guided lymph node surgery, tumor DNA sampling, immune therapy, etc etc and then you're facing treatment abroad and large sums.

In my case, it was all done in Bangkok (Bangkok Hospital), I spent north of 2.000$ on this thing from the initial dermascopy until the surgery and final pathology .
This includes a last minute rush trip to Bangkok and back and a five nights stay there.

Had one on my forehead, ( apx 1" round, 1/8th "deep) initial consultation, biopsy, surgery removal, ( they wanted to remove some skin from behind my ear graft it on, i opted to just let it heal on its on) follow ups and meds was 18,000 baht ( apx $ 600) at PPSI in the Bangkok Hospital Siriroj Phuket
excellent Drs and service
Had a few small ones removed from my arms where they freeze them & burn them off, 300 baht each

Melanomas 1/8th of an inch deep, over 3 mm, are considered extremely invasive (talking Breslow depth here, the thickness of the layer of cancer cells) and as per golden standard require much more than just surgery (second excision). Any melanoma with a thickness of over just 1 mm is considered dangerous. Yes, 1 mm. And melanomas are never just frozen and burnt off by real doctors. Benign moles and some other forms of (pre)skin cancer are.

@Kung-fu Hillbilly : where can I find this Catherine? I have never done a check before, but I have a certain amount of moles which after I read this thing should get checked by someone knowledgeable...
