I don't see online any examples of completed/proper (both sides of card) c.d.c. hard copy card for covid shots/records from u.s.a. ? Appears Cambodia wants paper printed copies both sides of card and will save you time on arrival ? Thankyou

That’s because there is no standard Covid vaccine shot card in the US. Just find a copier and copy both sides of the card you have.

Just a photocopy (of both sides to be on the safe side) and hand to MoH officials on arrival, they can't scan the cards with the scanners they use which is why they want a photcopy, if you don't have they will make a copy for you for $5.

I remember last August-- I happened to have had 2 copies of my Forte Insurance. 1 was in black and white-- the other color. They specifically wanted color and would of been an issue. Def want color then and assume they can make an issue of it
