I can contribute with the following info that I got from my Sala Sangkat;

Sangkat Chak Angre Leu: A8, Borey chip Mong Street 271.
Lieutenant Colonel Cheng Sokunthy, 012 21 2454 or Colonel Luy Samsak, 088 64 42 999, This military zone is in charge of Card Distribution. My card should be ready before the end of this month, I got my second shot on June 16.

Do you have a map position for the address? Sangkat Chak Angre Leu: A8, Borey chip Mong Street 271.

Street 271 is very long, the Sangkat Chak Angre only shows as a pagoda, Borey Chip Mong is miles away from that pagoda and neither of them are on street 271.

Does any county have a vac card , NZ still haven't they are thinking about how to do it, I just got a crap cardboard card with name hand written on it so requested letter from government of my medical records with vaccination , it came back with wrong dates on it for both vaccinations , had to request a new one with correct dates.
So Cambodia isn't that bad, at least you have the QR code.

2nd dose on 23rd May, still no news on the card.

Not sure name of location, somewhere close to where the train line branch out to connect to the airport (which is now removed).

Heard that the card issuer has only 1 printing machine. Each day can print out 4-5K cards. Of the millions who have been vaccinated in Phnom Penh, you do the calculation. Either you have the right connection or you have to give a little tips to expedite the process, if not just be patient. You should be getting your vaccine card before Christmas.

who cares? I've had both shots an never been asked to prove it>

Until travel starts AND the world accepts Chinese shots, ( my fear is it wont be) it really doesn't matter
