Road Trip North East / East Cambodia
Can you suggest destinations or interesting places to visit , ruins , river towns laid back places in this part of Cambodia . Example the Romulous ruins north of Svay Rieng , which I was unable to find last time . Or just relaxing scenic places to chill out for a few days , nature reserves etc . I think this area is less touristed than other parts of the country probably because theres less there so any ideas would be gratefully considered .

You didnt say how much time you have.
An interesting trip if you are coming from PP is Mondulkiri, Ratanakiri, Steung Treng, Kratie, Kampong Cham, and back to PP. You need a reasonable amount of time as there are long distances between places.
There is a waterfall in Mondulkiri, but not real big.
I suggest going to one or more tribal villages in Mondulkiri.
Even the tribal farming areas are interesting.
In Mondulkiri you can climb a hill/mountain and look out over Vietnam. But it may be difficult for tourists to find.
When I was in Mondulkiri I spent time with people from a local village, and they showed me around.
There is a nice lake in Ratanakiri, worth going to.
Another option is PP, Mondulkiri, Ratanakiri, Steung Treng, Siem Reap.

you'd need at least 7 days, preferable 10 with your own transport so can explore

Must see places in Ratanakiri are Yeak Loam lake, and most people visit Kachanh and Katieng waterfalls because they're within easy reach. There are more waterfalls around but they're more remote. Ta Chea bar / restaurant in Ban Lung is worth a visit if you want to sample some local food.
You could drive up north to Veun Sai or Ta Veng districts, on the shore of the Se San river. You can make trips to cemeteries of indigenous groups if you like it. Across the river there's also some forest left for a good hike, but access is getting more and more difficult because the Bong Thom's don't like people to look around too much there.
If you go this month or next month make sure you stop at one of the many cashew nut plantations. Most have never seen or sampled the fruit of the cashew nut and Jan, Feb and March are the months you can sample it. You can also buy fresh cashews and grill them yourself. It's a nice thing to do once. Be careful with the fluid under the skin protecting the nut though - in can be highly irritating to your skin.
If you have a car you could also go and see the 'twin lake'. It's a crater lake like Yeak Loam but it was cause by a double eruption. The lake is not as spectacular as Yeak Loam but still worth a visit. It's about 35 km from the provncial center. You have to take the main road to Borkeov and just after you pass Borkeov you turn right.

If you want less tourist routes providing you have transport and want a mixture of road covering, tarmac and very long stretches of dirt road, have a go at the top of the country, Preah Vihear and beyond.
From Siem Reap take road 64 towards Preah Vihear take the left fork on road 62 that will take you into Kulen Prum Tep Wildlife Sanctuary, carry on to Sra'aem and on to Angkrong near the border of Thailand, this is where you will find Preah Vihear Temple. From there it is possible to follow the Thai and then Laos border on the Cambodian side, dirt roads and heavy forests, then you will come to the falls that phuketrichard mentioned. Sopheakmit Waterfall, there is very good food at Waterfall view restaurant, after there you can head down river to see the Dolphins before Anlung Chheuteal, then on to Stung Treng.

I just came across this video which has some cool drone views of Sen Monorom and a road trip to Phnom Dos Kramom in Mondulkiri. It's from 2018. Enjoy the nature visit.
I was up in Mondolkiri about the same time, sometime in 2018,but was wondering about changes since then. Anyone ?
