What percentage of Americans actually cook from fresh ingredients at home?

Whenever I think of American food, I immediately think of McDonald's or Burgerking having only ever spent three and a half days in New York, but surely this isn't really what Americans eat on a daily basis?. Or is it?

What is the majorities diet like in the States?
How many people actually cook at home using fresh ingredients cooked in the main without the use of a microwave.?

microwave is only valid for re heating previously cooked foods.

I'm American, and I can't ever recall eating a meal with fresh ingredients while growing up. All vegetables were frozen, and most meals, be it pasta, burgers or meat, all came out of the box.

My first wife use to tell people that she loved cooking, it's so easy. Just open the tin and heat it up.

Vastly over-summarising, I found a huge amount of people I worked with ordered takeout because of 15-30 min lunchbreaks. Only those with stay at home wiges or husbands had home cooked/prepared food. I was the only single guy who prepared my own food most of the time.
