A vegetative state would imply oxygen starvation, a botched anaesthetic or just heart failure without timely resuscitation will do the trick. You wouldn't want to be that doctor who treated him.

I see no reason why his sister would be any less ruthless than he has been. In fact I can almost predict the memes that will be made using the tweets alluding to N.Korea having a female leader before the USA coupled to headlines of her having some power hungry adversary brutally executed.
In fact I predict she will be far worse than poor ol’ Rocket Man and will go on a rampage of removing any challenges to her authority. She will have to to keep a hold on power.
