Or can I just print out and fill in the blanks?

Huh? More context/details needed...ie. are you trying to get custody in Cambodia of your children born to a Cambodian mother? Is the mother still alive, or are you divorced/never married? Is your name on their official birth certs? Has the mother agreed to give you full custody? etc. etc...

i got mine , no paperwork needed , cops took the kid away from her and gave him to me
she still visits when she needs money ( often )
she cant afford my kid

they also revamped the whole birth certificate for me
