Thanks for the help. Unfortunately that phone no. is out of date and when I went in person it was super busy so I didn't even bother talking to anyone. I'll try going late at night just to get the phone no.
Vaccination starts early I went 8am Afternoon starts 2pm just show your card at front security desk and you will be stamped Go up to level 2 and present ur card to Dr at desk You will be directed to queue for vaccination After you will have your card updated May have a wait if a lot of people

As of the 15th November they were extending the booster shot programme in Phnom Penh indefinitely, but not sure if they are still in these places. Click on this Khmer Times article and it give a list of where to go, Khan by Khan

Cambodia purchases 4 million Molnupiravir tablets

Cambodia has arranged purchase of four million Molnupiravir pills – which is enough to treat 100,000 Covid-19 patients – even though the current rate of daily infections in Cambodia remains low.

The medicine will be distributed to state hospitals, pharmacies and private clinics, according to the Samdech Techo Voluntary Youth Doctor Association (TYDA), an organisation that has been heavily involved with treating Covid-19 patients.

In a Facebook Post on December 3, TYDA said its deputy chairman of the board Pich Chanmony – who is also a part of the government working group in charge of research on medicine and medical equipment to combat Covid-19 – signed a purchase contract to buy Molnupiravir from the US-based manufacturer, the pharmaceutical giant Merck.

TYDA stated that the new antiviral drug for use in treating Covid-19 was an important safeguard to help patients at risk of serious illness, which is why the Cambodian government purchased enough doses to treat 100,000 people.

“It’s good news for Cambodia,” said Pich Chanmony on Facebook.

HE announced the purchase of Molnupiravir on November 1, with a total cost of about $2.5 million.

China Pledges to Support Cambodia Establishes COVID19 Vaccines Manufacturing Factory

Phnom Penh (FN), Dec. 5 – China will further provide assistances to Cambodia in the fight against the COVID19, including the support to establish COVID19 vaccines manufacturing factory in Cambodia.

Chinese foreign minister Wang Yi told Cambodian foreign minister Prak Sokhonn during the latter’s visit to China’s Zhejiang province from 3 to 5 December 2021.

According to Cambodia’s Foreign Ministry, Prak Sokhonn expressed his appreciation for China’s commitment to increase the import of Cambodian agricultural products to the Chinese market. He reiterated Cambodia’s appreciation to China’s development assistance, which has significantly contributed to the socio-economic development of Cambodia, especially the provision of medical supplies and vaccines that are instrumental for Cambodia to put the pandemic under control.

In response, Wang Yi reaffirmed China’s strong commitment to continue to provide assistance to Cambodia in the fight against the COVID19, including the support to establish COVID19 vaccines manufacturing factory in Cambodia.

The two foreign ministers also appreciate the progress of the project implementation under the Belt and Road Cooperation in spite of the COVID19 pandemic.

Cambodia to Have Home-made COVID-19 Vaccine Next Year
AKP Phnom Penh, December 06, 2021 --

Cambodian people would be able to use the COVID-19 vaccines manufactured in Cambodia next year, underlined Chinese Ambassador H.E. Wang Wentian this morning.

While he and Cambodian Prime Minister Samdech Akka Moha Sena Padei Techo HE were presiding over the official inauguration ceremony of National Road (NR) No. 11 and Prey Veng city road with the total length of over 96 kilometres from Neak Loeung, Junction with NR No. 1, Prey Veng province to Thnal Totoeung, Junction with NR No. 7, Thbong Khmum province under China’s concessional loan, the Chinese ambassador said Sinopharm company has been working closely with the Cambodian Ministry of Health on this initiative.

China has been paying high attention to Cambodia’s request on the creation of a COVID-19 vaccine plant in the Kingdom, H.E. Wang Wentian added.

Prime Minister Samdech Techo HE said that the vaccines produced in Cambodia will be used domestically and for export. He further stressed that with local production of COVID-19 vaccines, Cambodia will feel secured that it has enough vaccines and it does not have to pay transport costs, and more job opportunities will be created.

The Premier took the opportunity to advise the Ministry of Health and governors of bordering provinces to check whether the forces deployed at the border have gotten their 3rd dose, and the 4th dose if needed, to ensure the safety of all frontliners.

In mid-November, Samdech Techo HE said Cambodia has been studying the initiative to manufacture COVID-19 vaccines in the Kingdom with raw materials from China, considering that vaccine is a global goods.

Ministry To Transfer the Distribution of Rapid Test Kits to Cambodia Post From December 13 Onwards
Cambodia News(Phnom Penh) According to the publication of the Ministry of Posts and Telecommunications on 8 December 2021, the Ministry will transfer the distribution of the rapid test kits currently distributed, (the Asan Easy Test RTK Antigen test kits made in Korea and Wondfo made in China), to Cambodia Post from 13 December 2021 onwards.
Members of the public who have ordered and paid before the change in distributors date, will need to collect the rapid test equipment at the office of the Ministry of Posts and Telecommunications.

With this change in distributors, members of the public including government and private institutions may continue to order the test equipment through the automated system website opened before, or make purchases directly at the Cambodia Post office located on Preah Ang Eng (13), Sangkat Wat Phnom, Khan Daun Penh, Phnom Penh.

I also failed to locate the app.
For Ministry of Defense cards, you can download a digital card after scanning the QR code of your physical card. The digital card contains all vaccinations, including boosters and looks pretty impressive.
