You do not need that form any more, it was for travel in PP during lockdown before the Yellow zones came in. There is now free movement.

You can go from Palace Gate (for example) to either the NIPH (2 day result, cost $130 - register first) or the Pasteur Institute (same day results, cost $75 - appointment required)

My friend is at Palace Gate and they will even make the booking and drive him there. Pretty good service.

The pre flight test has been covered in detail on the thread detailed below including the NIPH website for registration and the Pasteur Institute's email address.
