I would have thought that the authorities would require something specific concerning general health relating to the current situtation not a general questionnaire about ability st sit unaided, use the toilet and regulate their own oxygen supply.
Is their any evidence thst this declaration would work?

Thailand has closed all of its borders - land, sea and air - to foreigners trying to enter the country.

Thailand has closed all of its borders - land, sea and air - to foreigners trying to enter the country.
However, borders will remain open to Thais returning from overseas and to foreigners who are in possession of a valid work permit. Diplomats will also be able to still enter the country.
The partial closure of borders was one of a number of measures confirmed by the Thai government on Wednesday as part of its recently announced emergency decree.
Borders will be closed from 26 March 2020.

Thai Airlines announced on its website on 24th that as from the 25th it was suspending all its Asian flights including Phnom Penh. Tonight the 25th its Phnom Penh was still flying

He says that more stringent measures will be imposed that will affect their daily lives.
The State of Emergency is now in effect. The PM said that more stringent measures are necessary to slow the spread of Covid-19 and to reduce the socio-economic impacts from the contagion.
In recent days there has been rumours of a national curfew but, at this stage, none has been enacted. Locally, there was a curfew in Phuket last night, from 5pm-midnight, asking all tourists to remain in their accomodation. The announcement, from the Phuket Governor, was an effort to avoid a repeat of spontaneous gatherings on Patong Beach the night.

He added that the company will also enforce social distancing by leaving empty alternate seats on the buses and keep a distance of at least one metre between persons at all bus terminals.
After the state of emergency was announced on Tuesday (March 24), people rushed to buy bus tickets. All tickets have been sold out beyond April 30. “We cannot provide additional buses and all available buses have been used. Complying with the social distancing policy halves the number of passengers per bus,” added Jirasak.
