First, you have to remember that Americans are raised with the ideal that everyone is allowed to say whatever they want. Freedom of speech is considered to be a cornerstone of the culture like nowhere else on the planet. You are definitely not allowed to hit someone who is simply exercising their Constitutional right. Forget being sued. You would probably be thrown in jail for assault.
Secondly, that woman was quite clearly insane. You don't hit someone who is sick. You do your best to avoid them and either pawn the problem off onto someone else, or if that doesn't work, call the police and get them to come lock her up pending psychiatric evaluation.
