A divorce or a business mistake may have swallowed his savings. Insurance is expensive the older you get.

If he can make it back to the states Medicare should cover him.

He can possibly get Medicare Part A coverage (hospital), however if he did not keep his Part B payments up to date, he won't get out of hospital treatment covered (80/20) by Medicare.
