Same as Hanno, but I regularly get $50s from the Toul Kork ATMs - never had an issue with any note there. Plus knowing a few folks on the inside in the good old days, I'd put $50 to charity (don't like to talk about it), that this wasn't in the ANZ ATM - too many processes & procedures that bank, proper "belt & braces" stuff not like the other Noddy local banks. Again the ATM note drawer camera if installed on that ATM will most likely be able to prove that the money provided was legitimate & the ol' "switcheroo" did indeed happen elsewhere.

I'm now in the habit of withdrawing Riel instead of dollars. If it wasn't for a work colleague who'd withdrawn a fake $100 from an ATM (I won't name the bank) I'd find it difficult to believe that this sort of thing was really happening - at least happening as people are reporting it (in that fake bills are coming directly out of ATMs) but I know this work colleague very well and I have no reason to doubt that he genuinely withdrew a fake $100 bill.
As I've said before, brush up on your maths, stop being so lazy, learn your four times table, and stick to Riel. It's available from every ATM, is accepted everywhere, is available in much larger denominations than the change you're given in the shops, and does not get forged (as far as I'm aware).
You will also save yourelf a bit of money too. When you withdraw 400,000 Riel, which you would assume to be $100, the bank (or ABA does at least) only charges you $98.50 or thereabouts (it varies slightly from day to day as you'd expect).
Small saving I know, but every little helps

There are counterfeit Khmer Riel notes, right up to the 100,000.

Today for instance it is KHR 4,062 to $1. Thus, you withdraw KHR 400,000, divide that by 4,062 and you get $94.48 - probably less, as a bank will ALWAYS take margin on any FX conversion.
[note: if you want to test how generous your bank is - ask any restaurant to change your KHR 400,000 from the ATM into a crisp clean $100 bill]

Xandreu is right if you are paying in riel at markets etc, but once you are paying at Lucky/Thai Huot and probably most bars you'll find they charge an inflated rate of 4100 riel/$1 so you'll be worse off as the actual rate is usually around 4060. If things are really tight you could take advantage of this by using riel and dollars wherever you get the best deal. [Admin edit: don't bait other members. Thanks.]
