Randomised controlled trials have also shown that the effective praziquantel dosage regimen is 60 mg/kg orally in divided doses over one day (3 x 20 mg/kg doses 4-hourly or 2 x 30 mg/kg either 4-hourly or 6-hourly) for S japonicum and S mekongi, and 40 mg/kg orally in divided doses over one day (2 x 20 mg/kg doses 4-hourly) for S mansoni, S haematobium, and S intercalatum.1 7 19 22 23 24 (table(table).). However, a single 40 mg/kg dose is administered with 66-95% efficacy for epidemiological studies and population-based preventive chemotherapy control programmes.2w60 Efficacy of 95-100% can be achieved with re-treatment four to six weeks later25 and this second dose is advisable, especially if eosinophilia and a high antibody titre persist after primary treatment.

So I've been here awhile and I haven't been de-wormed yet. Should I just eat one Abendazole or something else as well ? What's a good plan ? Thanks in advance.

They get liver fluke from eating fermented fish (prahoc etc) that is not cooked before the process starts. There's a mass education program going on in Thailand to ensure that the fish is cooked first to kill the flukes before fermenting. Of course you could just avoid eating prahoc all together. I had bilharzia twice in Africa, they used ambilhar and later another drug by injection, the treatment made me puke for two days

Also Albendazole is generally better tolerated "Treatment with albendazole appears to be better tolerated, with fewer uncomfortable side effects like nausea and vomiting. If a patient had a bad reaction in the past to mebendazole, the other drug may be preferred to keep the patient more comfortable."

However Albendazole is not as effective against strongyloidiasis as Ivermectin, which is the drug of choice. The dose is 200 µg/kg orally for 1-2 days (6mg per 30kg bodyweight) Nb also the same dosage for treating Scabies.

Zentar plus is available here and contains both Albendazole 400mg and Ivermectin 6mg and is around 3000r per tablet.
Safety
Ivermectin is not recommended in children < 5 years because of a lack of data.11 Refer young children with strongyloidiasis to a paediatrician or infectious-disease specialist.3 In remote areas where access to a specialist may be difficult, albendazole may be given.5 Use 200 mg once daily for 3 days if the child is < 10 kg, and 400 mg once daily for 3 days if the child is > 10 kg. Take on an empty stomach to increase intestinal absorption and repeat after 1 week.4,9
Ivermectin should not be used in people who also have Loa loa (loiasis), as there is a risk of severe encephalopathy or death in these patients.3,11 Use serology to rule out L. loa in people from West and Central Africa (where it is endemic) or in people who report episodic subcutaneous swellings or an 'eye-worm'.3 Refer people with loiasis to an infectious-disease specialist.3
Albendazole should not be used in pregnancy or in children aged < 6 months.9,12 The manufacturer recommends discontinuing breastfeeding during, and for 5 days after, treatment.12

Unless you've got a tapeworm hanging out your arse I'd skip prophylactic/just-in-case de-worming treatment. Side effects weren't worth it in my experience.

once a year is good. the visible evidence of infection is pretty gross.
